Medical Withdrawal

When a student withdraws for medical (including psychological, psychiatric, or physical health) reasons, he or she must complete a Medical Withdrawal Agreement and have that agreement signed by the appropriate dean's office. Copies of the form must be sent to CAPS, the Student Health Center (SHC), and the treating professional.

Returning students

When a student who has withdrawn from the University due to medical reasons is ready to return, the student must communicate this in writing to the appropriate dean's office. In addition, the following forms must be downloaded, completed, signed, and faxed or mailed to CAPS (804-287-1227).

  1. A signed Personal Statement from the student.
  2. A signed Authorization form allowing the student's treating health professional to share confidential health information with CAPS and the SHC.
  3. A signed Authorization form allowing CAPS and the SHC to share relevant confidential health information with the appropriate UR dean's office.
  4. A signed Treatment Summary from the student's primary treating health professional (sent directly from the health professional to CAPS).
